Dr. Mark G. Brooks, MD

Dr. Brooks is Board Certified in Internal Medicine and is a member of the American College of Physicians. He earned his Bachelor’s of Science degree at the University of Florida and obtained his Medical Degree at the University of Miami. He completed his internship and residency at Orlando Health. He has served on the Medical Executive Committee and as Chief of the Department of Medicine at Orlando Health Central Hospital. He is currently an adjunct faculty member for the Orlando Health Internal Medicine Residency Program.

Antonio Velardi, M.D

Dr. Velardi come to Horizon Primary Care with many years as a Critical Care Medicine. He has served in such roles as Chief Clinical Officer, Chief Quality Officer, Director of Critical Care Medicine in various hospital, physician and clinical settings. He is Board Certified in Internal Medicine by the American Board of Internal Medicine. His education includes the University of Milan, Rutgers Robert Wood Johnson Medical School, Mercy Catholic Medical Center and St. Louis University. Dr. Velardi is fluent in Italian and proficient in Spanish. In his free time, he is learning to play piano!

Dr. Ian J Figueroa

Dr. Ian J Figueroa is a General Practitioner with a experience in primary care for over 14 years in the state of Florida. He is bilingual and earned his Medical degree at Central East University in San Pedro de Macorix, Dominican Republic and completed his internship at the Hospital San Gerardo in San Juan, Puerto Rico. Dr. Figueroa likes to spent time with his family and friends

Tom Pham, PA-C

Tom is a double “Gator.” He graduated with both his undergraduate and graduate degrees from the University of Florida. Tom enjoys Family Medicine and brings his experience in this field with him to Horizon Primary Care. Tom loves his 2 cats and 1 dog. When he isn’t zipping around here at work, Tom enjoys karate, running and watching movies.

Christopher Rotondo, PA-C

Chris received his undergraduate degree from University of Florida and later went on to get his graduate degree from Nova Southeastern University. He is a member of The American Academy of Physician Assistants and the Florida Academy of Physician Assistants. Mr. Rotondo had an article featured in The LIFE Study as part of the UF Health Promotion Center.

Jensine Caudill, FNP-C

Jensine received her Bachelor’s degree in Nursing at University of South Florida and later went on to receive her graduate degree at Purdue Global University. Jensine has worked on staff as a charge nurse and preceptor in both surgical/orthopedic care, preop/postop care, and progressive care units. She loves spending time with her family at home and at the local theme parks.

Terry Clemons, APRN-C

Terry received her undergraduate degree in Nursing at Florida Statue University and later went on to receive her graduate degree at the University of Central Florida. Terry has worked on staff as an adjunct faculty member at local colleges and at local hospitals as a charge nurse, preceptor and in case management. Terry also has been a volunteer at Shepherd’s Hope, a local free clinic, for many years.
